MOCEANS Support Coordination Services assists individuals living with a developmental or intellectual disability who are 21 or older and Medicaid eligible to achieve their highest level of independence.
What is Support Coordination?
Solutions-focused and Comprehensive Support Coordination
Support Coordination is a service offered through New Jersey’s Division of Developmental Disabilities. This service is paid through an individual’s Supports or Community Care Program budget to ensure access to services necessary to live as independently as possible in their community.
The Support Coordinator’s role is to listen to the individual and their supports in order to understand what they want and need in their life. Once that is understood, an Individualized Service Plan is developed that details the individual's hopes and dreams and the types of support they want in order to safely live the life they choose. Ultimately, MOCEANS Support Coordinators ensure the individual has access to services that encourages a life of contentment and continual thriving.
MOCEANS Support Coordinators are compassionate, dedicated professionals who develop solutions-focused service plans and advocate for each individual to achieve their highest level of independence.
